Instruction for MS-WORD format:
1. Press your right mouse
button (right click) on link.
2. On the popup menu, choose Save Target As .... it should be your 3rd
choice from the top for Internet Explorer 5.0. Or Save Link As...
your 4th choice from the bottom for Netscape 4.5.
3. Choose a location on your PC to save the file; for example save on your
Desktop. Then click on save button.
4. Double Click the file that you have just saved to your PC
5. Proceed to fill out the form.
